Care at the end of a life, at home
For the last weeks and days. Somebody in the house who is not frightened, who knows what to do with the ordinary things, and who is there so that no one has to be alone with it. Plainly
Somebody is dying, and you want it to be here
That is what this service is for. Not a decline, not a transition, not a journey. Somebody you love is going to die, and you have decided it should happen in their own bed with their own things around them.
It is a good decision and it is a heavy one, and most families making it have never done it before. What they are short of is not information. It is a competent person in the house.
So a caregiver comes and does the ordinary things. Washing and changing, gently and without hurry. Sheets. Mouth care and lips that get dry. Turning a pillow. Small amounts of whatever can still be managed, offered without pressure. The house kept going, so that nobody is doing laundry at midnight.
And sitting. A great deal of the work at this stage is sitting, awake, in a chair beside a bed, while a family sleeps or eats or goes outside for ten minutes without being afraid of what they will come back to.
This is non-medical care. Where hospice is involved they hold the licensed side of it, and our hours run alongside theirs rather than instead of them. Medication reminders only, not administration, at any stage and without exception.
Nights
Somebody awake, so that you can sleep
Nights are the part families are least prepared for and the part they remember longest. Cover is available overnight and around the clock, including weekends and holidays, and it can start the same week.
A caregiver on a night shift is not there to do very much. She is there so that the room is not empty at four in the morning. So that a daughter who has not slept properly in two weeks can go to bed in her own house and be woken if anything changes.
Families often say afterwards that the nights were what let them be present in the days. That is the honest case for it, and it is the only one we will make.
If you would rather not be alone in the house at the end, say so at the assessment. It is a normal thing to ask for and it is arranged more often than people imagine.
